Crock Pot Creamy Chicken Parmesan Soup

Ingredients:
1 lb boneless, skinless chicken breasts
4 cups chicken broth (low sodium preferred, adjust salt accordingly)
1 can (15 oz) diced tomatoes, undrained
1 teaspoon Italian seasoning
1 teaspoon garlic powder
1 teaspoon onion powder
½ teaspoon salt (or to taste)
¼ teaspoon black pepper (or to taste)
1 cup heavy cream
½ cup grated Parmesan cheese, plus more for garnish
8 oz cream cheese, softened (cut into cubes for easier melting)
1 cup chopped onion
2 cloves garlic, minced
1 cup shredded carrots
1 cup chopped celery
Optional: Red pepper flakes (for a little heat)
Optional: Fresh basil, chopped (for garnish)
Optional: Crusty bread or garlic bread, for serving
